Dealing With Chronic Conditions: Working With Peter

ADAPTING TO LIFE CHANGES
As a Clinical Health Educator and trainer, I counsel and support individuals with chronic conditions and their caregivers. Through Practical Dreamer, I offer classes, workshops and one-on-one consulting sessions to help you make life choices and changes that better manage your health, and maximize your quality of life.

Over the past 20 years I’ve worked one-on-one and in the classroom with thousands of people with health issues ranging from arthritis and asthma to diabetes, hypertension, tobacco addiction, weight management, headache management, stress-related symptoms more. I also have a specialized background in counseling and educating people with Fibromyalgia. 

What I’ve found over time is that there are certain common elements, tools and techniques  that are critical to optimizing your health with any chronic condition.  While I really enjoy doing customized health education one-on-one with my clients I’ve also distilled the core teachings into several simple, straight-forward classes and workshops for folks who enjoy the camaraderie and information sharing that goes on in a group setting.

My partner Joan Jackson works with me, bringing her own unique blend of stress management and energy management skills and knowledge to Practical Dreamer Chronic Conditions workshops and classes.

PRACTICAL DREAMER offers three streams of training and education for the management of chronic conditions:

-      Self-care workshops and education for people with a chronic conditions, including a specialize program on Fibromyalgia;

-      Caregiver workshops to support and educate anyone who takes care of a person with chronic conditions;

-    One-on-One sessions to assist you with your individual questions and needs.


 
WORKSHOPS AND PROGRAMS
 
Chronic Conditions:
TAKING GOOD CARE OF YOURSELF

The Chronic Conditions Self-Care Program consists of six two-hour class sessions over a six-week period.  We cover the topics here in slow, easy bites – meeting each week to learn, share issues and ask questions. This program is not only tremendously helpful if you have a chronic condition, but it’s also a great way for family members, and caregivers to learn.  Each session focuses on specific aspects of self-care.


 Self-care topics include:

·       Chronic illness basics

·       Communicating with physicians

·       Medications and symptoms

·       Physical activity and exercise

·       Pain management

·       Sleep hygiene

·       Healthy eating

·       Educating family and friends

·       Managing fatigue

·       Stress and relaxation skills

Caregivers' Workshops:
CARING FOR SOMEONE WITH CHRONIC CONDITIONS

The caregivers workshops are offered as all day or two-hour workshops.  The trainings are appropriate you if you’re caring for a member of your family, or if you’re a volunteer or professional caregiver.  The workshops focus on specific areas of information, skills, tools and the stress management for the caregiver.


Caregiver topics include:

·       Basics of chronic conditions

·       How to ask the doctor better questions

·       Tracking the care receivers medications

·       When and how to ask for help

·       Getting ready for a doctors visit

·       Stress management, pacing and self-care tools

·       Fundamentals on aging and medications

·       Dementia and Alzheimer’s Disease

·       Healthy diet and weight management

·       Living with Fibromyalgia

·       Physical activity and exercise

·       Basics of Arthritis, Hypertension, Diabetes and others.

LIVING BETTER WITH FIBROMYALGIA

Our Fibromyalgia Program consists of six two-hour class sessions similar in design to the chronic condition self-care program described above, the educational materials are specific to managing Fibromyalgia.  This program is also great  for family members, and caregivers.

While a Clinical Health Educator at Kaiser Permanente I lead the development and implementation of the Fibromyalgia Group Visit – a six-week program  for KP Northern California.  I also worked individually with many Fibromyalgia patients that needing additional support and help making lifestyle changes in order to successfully take control of their illness.

MORE ABOUT OUR CLASSES
We limit our private chronic conditions classes to a maximum of 15 participants with a minimum class size of six. Classes can be arranged on a group fee basis. Our caregiver workshops can accommodate groups from ten to sixty participants.  Fee schedule provided on request.

We also offer larger classes in collaboration with health agencies throughout Northern California.  See our home page for a list of upcoming workshops and classes.

FOLLOW-UP SERVICES

We offer a range of follow-up options to support learning and transition.

-      Assistance starting peer-led support groups

-     Intensives on specific areas of self-care including:

-          Developing a support system

-          Alternative treatments/resources

-          More about life-style change

-      One-on-one sessions on self-care


 

NO-FEE INTRODUCTORY
CAREGIVER WORKSHOP 

We offer a one-hour workshop on the Self-Care of Chronic Conditions to agencies, support and caregiver groups at no charge.  The presentation offers information about the three major goals of self-care: optimizing health status; managing the life changes inherent with chronic illness; and maximizing quality of life. The workshop includes ample time for questions and answers.

FEE SCHEDULE

-      A 1-hour face-to-face visit  $75

-      Phone visit – 40 minutes –  $60

-     Classes and  range in price depending on length and materials

-      If you’re from an agency that would like to offer support classes to seniors, people with chronic conditions or caregivers, please call Peter directly to discuss options

-      If cost is an issue, please call.  I keep a limited number of low-cost spaces available.
